Nevada Judiciary Threat and Violence Incident Reporting Instructions

Welcome to the Nevada Judiciary Threat and Violence Reporting System. This reporting system
is designed to document incidents involving potential threats or acts of violence within Nevada’s
courts. Collection of this data will help the Judicial Branch Security to better understand and
address safety concerns, in our courts, throughout the state.

This form is provided for Nevada court personnel to report threats or concerning conduct involving any person connected to court operations, including judges, court employees, jurors, and members of the public.

Nevada Judiciary Threat and Violence Report Form

* The date, time, and location of the incident.
* Whether an elected judge, judicial officer, staff member, or the public was involved.
* The nature of the threat or suspicious activity.
* If anyone was injured during the incident.
* The court district, investigating agency, and any related case numbers.
